• Observes and reports any changes of residents’ mental, physical or emotional status • Follows service plans and assignment sheets by reviewing current status of 24 hour shift and daily reports, completing training and team development courses, reading service plans • Provides routine updates and reports to supervisor after every shift • Responds urgently and promptly to emergency situations • Obtains and records residents' vital signs and weight, • Observes residents and reports adverse reactions to medication or treatment to medical personnel in charge • Collects samples, such as urine, blood, and sputum, from residents for testing. Prepares or examines food trays for prescribed diet and feeds residents • Records food intake, as required • Bathes, dresses, and assists residents in walking, transferring, and turning • Responds and follow up to resident pendants and inquiries and directs inquiries to the appropriate team member • Practices basic infection control precautions with linens, trash removal, resident change; wears mask, social distancing, cleaning and disinfecting and handwashing • Assists with meal service and light housekeeping, as required • Maintains working knowledge of fire and disaster plans, takes immediate action accordingly in situations • Promotes and Assists with resident activities as needed • Collaborates with team members to resolve concerns, problem solve interventions and preventions • Obsessed with customer service, reputation and commitment to positive outcomes • Completes all other duties and responsibilities as assigned
